CHANGING A CHEST TUBE DRESSING

_____1.	Verify order or check agency policy.
_____2.	Gather appropriate equipment (clean gloves, sterile gloves, Betadine swab, split dressing, 4x4 dressing, Vaseline gauze, 3” tape).
_____3.	Wash hands.
_____4.	Identify patient and explain the procedure.
_____5.	Open supplies, tear tape. Set supplies up in order you will apply them.
_____6.	Position patient, apply clean gloves, and remove old dressing.
_____7.	* Inspect site for redness, swelling, drainage, and crepitus. Do not touch tube.
_____8.	* Clean around insertion site with betadine swab x 1. (Wear clean gloves)
_____9.	* Apply sterile gloves without contaminating.
a.	* Apply sterile dressing. Apply Vaseline gauze around tube (if used), split dressing and 4x4. Tape securely making sure dressing is occlusive. (Dressing totally covered with tape). Anchor tube.
_____10.	Date and time dressing.
_____11.	Dispose of used supplies, wash hands.
_____12.	Chart procedure.
